The MDS is the backbone of how a skilled nursing facility documents resident needs and, in many cases, how it is reimbursed. Assessments must be completed and submitted within defined windows, and each window opens and closes on a calendar that moves every day as residents are admitted, change status or discharge.
For an MDS coordinator juggling dozens of residents, keeping all those dates in mind is a real challenge. A timeliness dashboard turns the calendar into a visible, shared tool so nothing slips. This post describes what to put on it and how to use it. Always refer to current CMS guidance and your compliance team for specific requirements.
Why timeliness matters
Assessments serve several purposes: they guide care planning, support accurate documentation and feed quality measures and payment. When an assessment is late or missed, the effects can ripple, from compliance concerns to payment impacts to gaps in the picture leaders use to manage care. Prevention is much easier than correction.
What a timeliness dashboard shows
Upcoming due dates
A forward-looking list of assessments due in the next several days, organized by resident, assessment type and the person responsible. This is the working view of the MDS team. It should be sorted so the most urgent items sit at the top.
Overdue and at-risk items
Anything past due or close to the end of its window should stand out clearly. Color coding works, as long as it is restrained and consistent.
Completion and submission status
An assessment is not finished when the clinical portion is done. Track the stages: in progress, completed, signed, submitted and accepted. A dashboard that shows where each assessment sits helps catch items stalled at a late step.
Error and rejection tracking
If submissions produce validation messages or rejections, track them and the time to resolve them. Recurring error types point to training or workflow opportunities.
Trend over time
Show on-time completion by month, assessment type and unit. Trends help leaders see whether process changes are working.
Who uses it, and how
MDS coordinators
They use the upcoming and at-risk views daily, planning work around the calendar.
DONs and unit leaders
They use it to see where clinical input is needed, such as a section waiting on therapy or nursing documentation.
Administrators
They use summary trends to confirm that the process is reliable and to spot staffing or workload concerns in the MDS department.
Billing and business office
They use it to understand when assessments that affect billing are expected, which supports accurate and timely claims.
Routines that make it work
- Daily check-in. A five-minute look at the next several days and any at-risk items.
- Weekly cross-team huddle. MDS, nursing, therapy and billing review the upcoming schedule and flag bottlenecks.
- Monthly trend review. Leaders look at on-time rates, rejection reasons and workload.
- Coverage planning. Make sure someone can cover the MDS function during vacations and sick days. A visible calendar makes handoffs easier.
Watch for workload signals
Timeliness problems are often workload problems. If the dashboard shows that assessments are clustering, as when a wave of admissions arrives at once, leaders can plan ahead and add support. Over time, a clear picture of volume helps justify staffing for the MDS function.
A hypothetical example
Picture a hypothetical building where the dashboard repeatedly shows several assessments reaching the end of their windows on Mondays. The team realizes weekend admissions create a Monday pileup and that the coordinator is out on Fridays. They adjust coverage and begin preparing weekend admissions earlier in the process. The dashboard did not do the work, but it showed where to look.
Keep the data trustworthy
A timeliness dashboard is only as good as its source data. Make sure dates and statuses come directly from your EHR rather than a manually maintained list, which can drift out of date.
CarePulse Analytics builds MDS timeliness views from PointClickCare and other EHR data so teams see deadlines in one place. If you would like to see one using your own assessment calendar, we can arrange a demo.
